Three Ways to Apply for a Card

Before submitting an application please read the following:

1. An applicant for a Hāpai Access Card  is required to provide evidence of their disability and information that will allow the allocation of appropriate barrier icons to their card.  This evidence can provided by one of the following:

a.  NGO:  An NGO that is registered with the Hāpai Foundation.

b.  Medical Professional:  A GP, or recognised medical services provider.

c.  Self:  By submitting medical information, and supporting to evidence through an online application process which will be assessed by a team at Nimbus Disability Consultants who have done over 30,000 such assessments.

2. At the very least you will require for an application:

a.  An electronic passport type photo ie jpeg.  This cannot be any old picture! The following site provides excellent info on how to take such a photo with either a Samsung, or iPhone cameras Photo Requirements. Your application will be delayed if you provide an inadequate photo.

b.  A means of payment through PayPal.  The cost is NZ$30, the card is valid for 3 years.

3.  NGO.  If you are a client of an NGO we suggest that you first approach them and ask if they can assist you.  If they are registered with the Hāpai Foundation it will make the process simpler as they can vouch that you are classified as disabled, and that you have certain barriers.  They will need to complete the "Validation of Disability and Barriers Form"  which can downloaded.  Once this is done you will be able to apply online via the Supported Application Process.   You will need a scanned copy of the completed "Validation of Disability and Barriers Form", an electronic passport photo ie jpeg, and a means of payment via Paypal.

4.  Medical Professional.  If you are not associated with an NGO, or the NGO you are associated with is not registered with the Hāpai Foundation,  you will need to get a GP, or recognised medical services provider eg Burwood Paediatrics Rehabilitation Services, to complete the "Validation of Disability and Barriers Form" which can downloaded

Please note: we do not require any medical information, and would request that none is included in this form as it will be immediately deleted as per our Privacy Statement. Once this is done you will be able to apply online via the Supported Application Process.   You will need a scanned copy of the completed "Validation of Disability and Barriers Form", an electronic passport photo ie jpeg, and a means of payment via Paypal.

5.  Self:  If you are not associated with an NGO, and do not want to use a GP, or other recognised medical provider, then you can make your application using the online Self Application Process. You will be required to provide medical evidence, which is securely stored and handled.  You will  also need an electronic passport photo ie jpeg, and a means of payment via Paypal.
