Registered NGOs Supporting Applications

An NGO, Trust or Medical Organisation can request to be registered with the Hāpai Foundation to be able to support a client in making a "fast track Application".   

 

To be registered we need to be confident that the organisation is established and able to certify that the person is disabled, plus can competently select out of the nine barriers which ones apply to the applicant.

 

Registered organisations agree that:

  • Anyone completing the form has read the information on the Guidance page of our website.

  • That a person competent to assess the applicant completes the form, and that this process is overseen by a designated manager.  

  • That the organisation will cooperate with any request by the Hapai Foundation to audit the process, and applications.

Please note that the "fast track application" does not require disclosure of medical information, and we specifically request that no such information is passed to us.

 

The process for a "fast track application"  is as follows:

 

Step 1:  The registered NGO completes the "Validation of Disability and Barriers" form -  (Download) including their authorisation code, and a promocode if appropriate.  This paper copy is scanned.  If the applicant is completing the form themselves then the scanned form is emailed to the them by the NGO as an attachment together with instructions (Download Instructions for Applying for a Hāpai Access Card with a “Validation of Disability and Barriers” Form).

HOT TIP - Free conversion of photo to passport size:  From a computer go to persofoto to either upload a photo for conversion to a passport sized one, or take one with the webcam.  Alternatively, from a smartphone go to the same site and either upload a photo on the phone, or take one there and then.  

Step 2:  (Note: Step 2 is conducted by the NGO/Trust where the applicant is not able to make the application themselves).  The applicant receives the email from the NGO with the pdf form and instructions.  They follow the instructions which has them complete the online application form which is found at the "applying for the card" webpage.

 

Payment can be made as part of the online application, but alternatives are to do a bank transfer, or to drop off the cash to our office at Kilmarnock Enterprises.  Contact us for details.
